Thank you for your recent visit to our office to meet with Espe Avalos and representatives
of City Departments to discuss your upcoming community health fair to be held May 31,
1997.
As a follow up to the meeting, I would like to summarize the required authorizations and
permits applicable to your event:
♦ Police Special Events Street Closure Permit $100.00
♦ Fire Department Assembly Permit 50.00
♦ Public Works Use of Street Permit 402.00
TOTAL $884.00
It is our understanding that you have requested fee waivers from the City Commission, and
that this item will be brought up as a pocket item during the City Commission uneeticig 01*
May 22, 1997. However, we must inform you that the City's current policy is not to
grant any fee waivers due to severe fiscal constraints, and in that case, it will be
necessary that you pay the City of Miami for all applicable permits and fees.

liability established by the City, and provide a current certificate of insurance naming the
City of Miami as additional insured.

Thank -you very much for time "prompt attention to our inquiries. We are pleased that you
share our interest in healthcare education needs in our community.
As discussed, The Miami Times is requesting your assistance as we present the first in a series of
community health fairs. The faies entitled, "Community Check-up," will address high incidence
disease in minority communities through education and preventive cure, The day -long event set
for Saturday, May 31 st is being developed in conjunction with The American Heart Association,
The American Cancer Society, The University of Miami, School of Medicine and area medical
centers.
Your assistance in introducing the event to the commission, for use of Miami streets, as outlined
on the attached, in your May 7th Commission meeting would be most appreciated as you join our
health awareness alliance. Our event is being hosted and supported by the City off ian*tiberty
City, Police Neighborhood Enhancement Team.
The Miami Times is working to curtail the high incidence of disease in our community through
education within a monthly healthcare supplement and four (4) community health fairs across
Dade County. Your support in this premiere "Community Check -Up," is a statement to your
commitment for the development of our city through education and collective effort of
cor nunity organizations.

sionals meeting the needs of the

Miami is the mecca for medi-
cal care with the second largest
hospital complex in the nation -

ties, such as The Sylvester Com-
prehensive Cancer Center,

MWAIDS, cancer, infarct
mortality, cardiovascular disease,
cirrhosis of the liver; and other
diseases which show a high inci-
dence in minoritty communities.

The specific needs of the community will
be addresses quarterly at locations across

testing and preventive care programs are
