ONE, A SUBDIVISION IN THE CITY OF MIAMI, SUBJECT TO ALL OF THE CONDITIONS OF THE PLAT AND STREET COMMITTEE, AND ACCEPTING THE DEDICATIONS SHOWN ON SAID PLAT; AUTHORIZING AND DIRECTING THE CITY MANAGER AND CITY CLERK TO EXECUTE THE PLAT; PROVIDING FOR THE RECORDATION OF SAID PLAT IN THE PUBLIC RECORDS OF MIAMI-DADE COUNTY, FLORIDA. WHEREAS, the City Department of Public Works recommends the acceptance of the plat; NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The plat entitled PHT/JMH Mental Health No. One is a replat of Tracts A and B, Medical Subdivision, Plat Book 129 at Page 76, and all of that part of the West 20 feet of N.W. 8 Avenue lying adjacent to said Tract A, all lying in Section 35, Township 53 South, Range 41 East, City of Miami, Miami -Dade County, Florida, which plat by reference is made a part hereof as if fully incorporated herein, and subject to all of the conditions required by the Plat and Street Committee as set forth as Exhibit "A11 attached hereto, is hereby accepted. The dedications shown on the plat together with the dedications to the perpetual use of the public of all existing and future ATTACHMENT (S)' CONTAINED SEP $ tm 98- 838 planting, trees and shrubbery on said property, are also hereby accepted and confirmed by the City of Miami, Florida. Section 2. The City Manager and City Clerk are hereby authorized and directed to execute the plat and cause the same to be recorded in the Public Records of Miami -Dade County, Florida. Section 3. Stierheim City Manager �xLiGi� i9„ Attention: Rolando Gonzalez Architectural and Construction Services Ladies and Gentlemen: RE: PHT/JMH Mental Health No. One - Tentative Plat 1487-B The City of Miami Plat and Street Committee, at its meeting of October 3, 1996, approved the above plat subject to the following revisions being made to the tentative plat, additional information being provided and/or variances being granted . Please be advised that the processing of your tentative plat cannot proceed until these conditions have been satisfied. • Section 54.5-16 requires that all property owners on both sides of the street to be vacated must join in the plat. The City Attorney's Office will provide an: -opinion to determine if the abutting owners must join in the plat in this particular case. • Provide an opinion of title to also include verification of all property owners who must sign -off on plat. This requirement depends on the results of the aforementioned legal opinion. • Provide the area (in square feet) of the parcel to be vacated. • Provide a legal description of the parcel to be vacated. • A fractional description must be shown in the location sketch. • The adjacent subdivisions south of N.W. 15 Street must be shown on the main sketch. • The requirements for water mains and hydrants remains the same as required in the original tentative plat. • Reflect Special District and Governmental Institutional zoning designations as described in Zoning Ordinance 11000, as amended, in surveyor's notes. Box 330708 Miami, FL 33233-0708/(305) 416-1 200/ Fax:f305) 416-219 8 838 Jackson Memorial Hospital Tentative Plat #1487-B October 16, 1996 • The plat and street committee has reviewed Section 54.5-16 with regard to the public interest requirements for the vacation and closure of a portion of N.W. 8 Avenue and several easements shown on the tentative plat. The committee has found that these requirements have been met and recommends the vacation and closure of the westerly 20 feet of N.W. 8 Avenue and the following easements: -15-foot wide north -south sewer easement, dedication found in Plat Book 129 at Page 76. -10-foot wide east -west easement, dedication found in Plat Book 129 at Page 76. -20-foot wide north -south Southern Bell Telephone easement, dedication found in Plat Book 129 at Page 76. In addition to the above requirements, you should be aware of the following: 1. State and local laws require the installation of various physical improvements in the public rights -of -way when property is platted. These subdivision improvements include paving, drainage, landscaping, sidewalks, etc. In some cases this could represent a substantial investment on your plat. 2. The alternation, relocation or installation of utilities such as storm and sanitary sewers, electric, telephone, water, etc., caused by this plat will be at the property owner's expense. Also, utility easements may be required on the property being platted. 3. A building permit will not be issued on the property being platted until the final is recorded. Also, the Certificate of Occupancy for any building construction will be only after all the required subdivision improvements have been completed. 4. Approval for fire flow requirements must be obtained from the Fire, Rescue and Inspection Services Department prior to the issuance of a building permit. 5. Additional items must be provided to the City of Miami Department of Public Works before the final plat is submitted to the City Commission approval. You will be notified in writing as to what these items are after the amount of the bond has been determined for the necessary subdivision improvements. It is in correct form for submission to the City Commission and is forwarded with a recommendation that it be approved.
