A RESOLUTION OF THE MIAMI CITY COMMISSION ACCEPTING THE RECOMMENDATION OF THE CITY MANAGER TO APPROVE THE FINDINGS OF THE EVALUATION COMMITTEE TO ACCEPT THE PROPOSAL OF MERCY MEDICAL DEVELOPMENT D/B/A MERCY OUTPATIENT CENTER TO PROVIDE. EMPLOYMENT AND ANNUAL PHYSICAL EXAMINATIONS AND HEPATITIS B IMMUNIZATION (PART I AND PART II SERVICES) FOR DESIGNATED PERSONNEL, AT A PROPOSED ANNUAL COST OF $744,923 PER YEAR FOR THE INITIAL TWO YEAR PERIOD, FOR THE DEPARTMENT OF HUMAN RESOURCES; AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE AN AGREEMENT, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, FOR AN INITIAL TWO (2) YEAR PERIOD WITH THE OPTION TO RENEW FOR TWO (2) ADDITIONAL ONE (1) YEAR PERIODS AT THE SAME TERMS AND CONDITIONS; AUTHORIZING AN INCREASE NOT TO EXCEED 5% IN THE AMOUNT OF COMPENSATION WHICH SHALL BE EFFECTIVE ON THE FIRST DAY OF THE FIRST OPTION PERIOD AND REMAIN IN EFFECT THROUGH THE REMAINING TERM OF THE CONTRACT; ALLOCATING FUNDS THEREFOR, FROM THE GENERAL OPERATING BUDGETS OF THE DEPARTMENT OF HUMAN RESOURCES, ACCOUNT CODE NO. 001000.270101.6.260; SPECIAL PROGRAMS AND ACCOUNTS; ACCOUNT CODE NO. 001000.921202.6.260; DEPARTMENT OF FIRE -RESCUE, ACCOUNT CODE NO. 001000.280401.6.260, AND DEPARTMENT OF POLICE, ACCOUNT CODE NO. 001000.290201.6.260; FURTHER AUTHORIZING THE CITY MANAGER TO INSTRUCT THE CHIEF PROCUREMENT OFFICER TO ISSUE A PURCHASE ORDER FOR THESE SERVICES., SUBJECT TO THE AVAILABILITY OF FUNDS. WHEREAS, pursuant to public notice, sealed proposals to provide services including employment and annual physical examinations and Hepatitis B Immunization (Part I and Part II services) for designated personnel, on a contract basis for two AUG 9 2 1999 Resolution No. (2) years, with the option to renew for two additional one (1) year periods, for the Department of Human Resources were received on June 21, 1999; and WHEREAS, the proposals were evaluated by an evaluation committee which recommended that the proposal received from Mercy Medical Development, d/b/a Mercy Outpatient Center, be accepted as the proposal most advantageous to the City; and WHEREAS, funds for these services are available from the General Operating Budgets for the Department of Human Resources, Account Code No. 001000.270101.6.260; Special Programs and Accounts; Account Code No. 001000.921202.6.260; Department of Fire -Rescue, Account Code No. 001000.280401.6.260; and Department of Police, Account Code 001000.290201.6.260; and WHEREAS, THE City Manager and the Director of Human Resources recommend that the proposal received from Mercy Medical Development, d/b/a Mercy Outpatient Center, for said services, be accepted as the proposal most advantageous to the City. NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI, FLORIDA: Section 1. The recitals and findings contained in the Preamble to this Resolution are hereby adopted by reference 0 thereto and incorporated herein as if fully set forth in this Section. Section 2. The proposal of Mercy Medical Development, d/b/a Mercy Outpatient Center, to provide employment and annual physical examinations and Hepatitis B Immunizations (Part I and Part II services), for designated personnel, at an estimated cost of $744,923 per year for the initial two year period, and allowing for an increase in compensation not to exceed 5% effective the first day of the first option period and remaining in effect thorough the remaining term of the contract, for the Department of Human Resources, is hereby accepted, with funds therefor hereby allocated from the General Operating Budgets of the Department of Human Resources, Account Code No. 001000.270101.6.260; Special Programs and Accounts; Account Code No. 001000.921202.6.260; Department of Fire -Rescue, Account Code No. 001000. 280401. 6. 260, and. Department of Police, Account Code No. 001000.290201.6.260. Section 3. Miami Avenue, Miami, Florida 33133, a local, non -minority proposer, to provide employment and annual physical examinations and a Hepatitis B Immunization Program (Part I and Part II services) for designated personnel for the Department of Human Resources at a proposed annual cost of $744,923. The contract would be for an initial two-year period with the option to renew for two additional one (1) year periods. BACKGROUND In an effort to secure a broad range of providers, a Request for Proposals (RFP) was issued for a qualified proposer(s) to perform employment and annual physical examinations and a Hepatitis B Immunization Program for the City of Miami. Eleven (11) firms were mailed the RFP. Section 2-268 of the City of Miami Code mandates the Department of Human Resources to establish and maintain medical services which shall include a medical examination of employees and potential employees. In addition to physical examinations, the RFP incorporated a Hepatitis B Immunization Program and various other medical services such as x-rays, EKGs, stress testing and in-depth audiological screenings. Two (2) sealed proposals were received on June 21, 1999. One of the proposals was deemed non -responsive as the facility was not located within the City of Miami limits as required in the RFP. The evaluation committee evaluated the remaining proposal deemed responsive and responsible for the provision of Part I (General Physical Examinations), and Part II (Police and Fire Physical Examinations, and Hepatitis B Program). The committee did not evaluate Part III (Drug Screening) services because it was deemed non -responsive as the facility was not certified in accordance with the requirements of the RFP. Based on an evaluation by two private sector professionals and staff from the Department of Police, it is recommended that the City Commission accept the findings of the evaluation committee and authorize the City Manager to enter into negotiations with Mercy Medical Development d/b/a Mercy Outpatient Center, to execute a contract in a form acceptable to the City Attorney, for the provision of Part I and Part II services, in an amount not to exceed $744,923 annually during the initial two (2) year term of the contract. An increase in the annual amount of compensation for the contract following the initial term shall be allowed in an amount not to exceed 5%, which shall be effective on the first day of the first option period and remain in effect through the remaining term of the contract.
