AFFIRMATIVE VOTE OF THE MEMBERS OF THE MIAMI
CITY COMMISSION, AFTER A DULY ADVERTISED
PUBLIC HEARING, RATIFYING, APPROVING, AND
CONFIRMING THE CITY MANAGER'S FINDING OF SOLE
SOURCE; WAIVING THE REQUIREMENTS FOR
COMPETITIVE SEALED BIDDING PROCEDURES AND
APPROVING THE PURCHASE OF LIFEPAK 12
DEFIBRILLATION/MONITORS, LIFEPAK 12 UNITS,
12-LEAD EKG MONITORS, AND RELATED EQUIPMENT
FROM PHYSIO CONTROL CORPORATION, THE SOLE
SOURCE PROVIDER OF THIS EQUIPMENT, FOR THE
DEPARTMENT OF FIRE -RESCUE, IN AN AMOUNT NOT
TO EXCEED $141,725; ALLOCATING FUNDS THEREFOR
FROM THE CAPITAL IMPROVEMENT PROGRAM, PROJECT
NO. 313304, "FIRE -RESCUE EQUIPMENT", ACCOUNT
NO. 289401.850.
WHEREAS, the Department of Fire -Rescue has used various
medical equipment provided by Physio Control Corporation, as a
sole source provider, since 1984 and has also maintained an
equipment maintenance and service agreement with said firm; and
WHEREAS, said Department now requires LifePak

Defibrillation/Monitors, LifePak 12 units, 12-lead EKG monitoring
options, and related equipment; and
WHEREAS, extensive field tests of comparable equipment on
the market have conclusively proven the superior function and
reliability of the Physio Control products; and
WHEREAS, the required equipment and accessories are fully

compatible with the systems now in use by the Department of Fire -
Rescue; and
WHEREAS, the Chief Procurement Officer has adopted a finding
that Physio Control is the sole source vendor capable of
providing the necessary equipment; and
WHEREAS, said sole source finding has been approved and
adopted as the finding of the City Manager; and
WHEREAS, the City Manager and the Director of the Department
of Fire -Rescue recommend that the requirements for competitive
sealed bidding procedures be waived and the purchase of said
equipment from Physio Control Corporation, the sole source
provider, be approved; and
WHEREAS, funds are available from the Capital Improvement
Program, "Fire -Rescue Equipment", Project No. 313304, Account
Code No. 289401.850;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the
Preamble to this Resolution are hereby adopted by reference
thereto and incorporated herein as if fully set forth in this
Section.
Section 2. By a four -fifths (4/5ths) affirmative vote,
the members of the City Commission hereby ratify, approve and
confirm the City Managers finding of sole source, waive the
requirements for competitive bidding procedures, and approve the
purchase of LifePak 12 Defibrillation/Monitors, LifePak 12 units,
12-lead EKG monitoring options, and related equipment from Physio

$141,725, With funds therefor hereby allocated from the Capital
Improvement Program, Project No. 313304, "Fire -Rescue Equipment",

Section 3. This Resolution shall become effective
immediately upon its adoption and signature of the Mayorll.

It is hereby requested that the City Commission adopt the attached Resolution, by a
four -fifths (4/5ths) affirmative vote of the Members of the City Commission after a duly
advertised public hearing, ratifying, approving, and confirming the City Manager's
finding of sole source; waiving the requirements for competitive sealed bids and
approving the purchase of fourteen (14) LifePak 12 Defibrillation/Monitors, fourteen
(14) LifePak 12, 12-Lead EKG monitoring options and related equipment from Physio
Control Corporation, a non-local/non-minority vendor, located at 11811 Willow Road
NE, Redmond, WA 98073, for the Department of Fire -Rescue. The proposed cost is
$141,725.00. Funding is available from the CIP Project entitled "Fire -Rescue
Equipment", Project No. 313304, Account Code 289401-850.
BACKGROUND:
The Department of Fire -Rescue has been utilizing equipment provided by Physio Control
Corporation pursuant to Resolutions 84-656 and 88-801 for repair and maintenance
services of the Physio Control Cardiac Life Support Equipment since at least 1984. The
Department of Fire -Rescue has conducted an extensive field test which involved the
major vendors of this type of equipment. The test results proved that the Physio-
Control product is superior in function and reliability.
Since all Department of Fire -Rescue cardiac monitoring and defibrillation equipment is
manufactured by Physio-Control, the Lifepak 12 equipment proposed for purchase is
fully compatible with all accessories currently in use within the Department. These
accessories include: EKG cables, lead cables, defibrillation pads, pacing pads, batteries
and battery support systems. This compatibility is vital to the smooth transfer of
patient care from one City of Miami Department of Fire -Rescue unit to another.
As the Chief Procurement Officer has deemed, Physio-Control Corporation as a sole
source; it is therefore recommended at this time that the City Commission ratify the
City Manager's finding of sole source and authorize the purchase of this equipment.
This expenditure is funded by the Fire Assessment.

An investigati9h was conducted by my staff to determine whether Physio Control Corporation, a non-minority/non-local
vendor located at 11811 Willow Road NE, Redmond, Wa. 98073, is the sole source provider for the acquisition of
LifePak 12 Defibrillation/Monitors and 12-Lead EKG Monitors with related equipment, for the Department of Fire -
Rescue.
The City of Miami has been purchasing various medical equipment from Physio Control Corporation as a sole source
provider since at least 1984, and has maintained a sole source maintenance and service agreement with that firm to
maintain said equipment during this period of time. The Department of Fire -Rescue is now seeking to purchase fourteen
(14) LifePak 12 Defibrillation/Monitors, fourteen (14) LifePak 12, 12-Lead EKG Monitoring options, and related
equipment. The acquisition of the LifePak 12 is an upgrade from the LifePak 11 currently used by the Department and
represents the next generation in acute cardiac care systems. The LifePak 12 is lighter, more user friendly, and guaranteed
to be Year 2000 compliant. The Department of Fire -Rescue will receive a $110,500 trade-in allowance on its thirteen
(13) used LifePak 11 units acquired previously through a sole source acquisition(s). In addition to the cash savings; the
new units all will have a one-year warranty.
As all the Department's cardiac monitoring and defibrillation equipment is manufactured by Physio-Control, the LifePak
12 equipment is fully compatible with all accessories currently in use within the Department, and the compatibility is
vital to the smooth transfer of patient care from one Fire -Rescue unit to another
Physio Control is the manufacturer and sole source provider for the LifePak 12 monitor defibrillator. No other monitor
defibrillator is compatible_ with the LifePak defibrillation/monitor which is currently being used by the Department of
Fire -Rescue. As a result, Physio Control is the sole source provider for the acquisition of the new generation of monitor
defibrillator and is fully compatible with the existing equipment being used by the Department for many years.
The proposed cost for the acquisition of fourteen (14) LifePak 12 Defibrillators/Monitors and fourteen (14) LifePak 12,
12-Lead EKG Monitors is $252,225, however, the Department will receive a $110,500 trade-in allowance on its thirteen
(13) used LifePak 1 I units. The total cost to the Department is $141,725.00, which includes a one year warranty. Funding
is available from the CIP Project entitled "Fire -Rescue Equipment," Project No. 313304, Account Code 289401-850.
Therefore, I am recommending that the requirements for competitive bidding be waived and the above findings be
approved: that Physio Control Corporation, a non-minority/non local vendor located at 11811 Willow Road NE,
Redmond, Wa. 98073 is the sole source provider for the provision of LifePak 12 Defibrillation/Monitors & 12-Lead EKG
Monitors, and related equipment, on behalf of the Department of Fire -Rescue.

Budgetary Impact Analysis of the purchase of Physio-Control LifePak 12
Defibrillators/Monitors and 12-Lead EKG Monitors
The purchase of the above mentioned Physio-Control Cardiac Monitoring
equipment will allow the Department of Fire -Rescue to upgrade from the LifePak
11 to the LifePak 12. The LifePak 12 is lighter, more user friendly and
guaranteed to be Year 2000 compliant.
The Department of Fire -Rescue will receive a $110,500 trade in allowance on its
thirteen 0 3) used LifePak 11 units.
This trade in allowance will allow the department to obtain $252,225 worth of
equipment for $141,725.00. In addition to the cash savings, the new units all
have a one year warranty, which will provide some financial relief on the
department's Physio-Control Maintenance Contract.
The LifePak 12 represents the next generation in acute cardiac care systems.

The following are some unique features that are specific to the Lifepak 12 monitor
defibrillator:
1. The Lifepak 12 has an integral diagnostic 12 lead ECG feature with interpretation,
100mm ECG printout and ability to transmit information to receiving facility
2. Pulse Oximetry and other modules will be available to assist in patient assessment and
treatment such as non-invasive blood pressure and end tidal CO2 monitoring.
3. This equipment is compatible with batteries and chargers, ECG cables, speciality
electrodes and paper already used within your system
4. Physio-Control field service maintenance provides in-house service within 24 hours
with loaner equipment provided
5. This equipment has the largest screen available with multiple patient parameter
information available for easy viewing
6. The Lifepak 12 is capable of mounting to your stretchers for ease of patient transfers
7. It includes both a manual and automatic external defibrillator
8. You are receiving substantial trade-in values on current equipment
Physio-Control is the sole vendor of the Lifepak 12 monitor defibrillator and provides
certified local field service technicians.

$252,225, however, the Department will receive a $110,500 trade-in allowance on its thirteen (13) used LifePak 11 units.
