RESOLUTION NO.
A RESOLUTION OF THE MIAMI CITY COMMISSION,
WITH ATTACHMENT(S), AMENDING RESOLUTION NO.
99-253, ACCEPTING BIDS FROM ALINA SPORTSWEAR,
INC., PENTAGON HEALTH CARE SERVICES,
EMBROIDER IT, INC., ZIPP SPORTING GOODS,
CREATIVE HANDS, INC., AND MIAMI TO GO, INC.,
FOR THE FURNISHING OF T-SHIRTS AND GOLF
SHIRTS ON A CITYWIDE, CONTRACT BASIS, AT A
TOTAL ANNUAL AMOUNT NOT TO EXCEED $85,000, BY -
RESCINDING THE BID AWARD FROM CREATIVE HANDS,
INC. AND ACCEPTING THE BID OF RDH QUEST,
INC., FOR THE FURNISHING OF SAID SHIRTS;
FURTHER INCREASING THE TOTAL CONTRACT AMOUNT
BY $30,000, FROM $85,000 TO $115,000;
ALLOCATING FUNDS THEREFOR FROM VARIOUS USER
DEPARTMENTS AT TIME OF NEED, SUBJECT TO.
BUDGETARY APPROVAL.
WHEREAS, Resolution No. 99-253, adopted April 27, 1999,
accepted bids of Alina Sportswear, Inca Pentagon Health Care
Services, Inc., Embroider It, Inc., Zipp Sporting Goods, Creative `
Hands and Miami To go, Inc., for the citywide procurement of
T-shirts and golf Shirts, on an as needed contract basis for one
year, with the option to renew for two additional one-year
periods, at an annual amount not to exceed $85,000, with funds
therefore hereby allocated from the user department budgets at
time of need; and
WHEREAS, Creative Hands, the low bidder for Item No. 11 of
the contract failed to provide the shirts; and

WHEREAS, the Departments of Fire Rescue and Purchasing made
several attempts to contact Creative Hands but the vendor failed
to respond to the requests for service; and
WHEREAS, RDH Quest, Inc. submitted the second lowest bid for
Item No. 11 of the contract; and
WHEREAS, the Department of Purchasing sees a greater need
for shirts from user departments; and
WHEREAS, an increase in the amount of $30,000, is necessary
in order to meet the increasing demand for the shirts; and
WHEREAS, funding for this increase is available from the
various user department budgets at time of need, subject to
budgetary approval; and
WHEREAS, the City Manager and the Director of Purchasing
recommend that award be made to RDH Quest, Inc.;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:
Section 1. The recitals and findings contained in the
Preamble to this Resolution are hereby adopted by reference
thereto and incorporated herein as if fully set forth in this
section.

Section 2. Resolution No. 99-253, adopted
April 27, 1999, is hereby amended in the following particulars:l/
Resolution 99-253
"Section 2. The February 22, 19991 bids submitted
by Alina Sportswear, Inc. Pentagon Health Care
Services, Inc., Embroider It, Inc. Zipp Sporting
Goods, Greative Hands RDH Quest, Inc. and Miami To go,
Inc., are hereby accepted, for the Citywide
procurement of T-shirts and golf shirts, on an
as -needed contract basis for one.year with the option
to renew for two additional one year periods, at an
annual amount not to_ exceed $85,000 $115,000, with
funds therefor hereby allocated from the user
department budgets at time of need.
Section 3. This Resolution shall become effective
immediately upon its adoption and signature of the Mayor./

words and/or figures shall be added. The remaining provisions are now
in effect and remain unchanged. Asterisks indicate omitted and
unchanged material.
If the Mayor does not sign this Resolution, it shall become effective at
the end of ten calendar days from the date it was passed and adopted.
If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.

ENCLOSURES:
It is respectfully recommended that the City Commission adopt the attached Resolution amending
Resolution No. 99-253, 1) Rescinding award to Creative Hands, Inc., 2) Awarding bid to RDH Quest,
Inc., a Local/Black owned vendor, located at 130 NE 40 Street., #9; and 3) Increasing the contract
amount by $30,000.00 for a new total annual contract amount not to exceed $115,000.00 for the purchase
of T -Shirts and Golf Shirts with Silk -Screening and Embroidery Citywide,. Funds are allocated from the
various user department budgets at time of need.
BACKGROUND
Resolution No. 99-253, adopted April 27, 1999, accepted the bids of Alina Sportswear, Inc. Pentagon
Health Care Services, Inc., Embroider It, Inc. Zipp Sporting Goods, Creative Hands and Miami Togo,
Inc. for the purchase of T -Shirts and Golf Shirts with silk-screening & embroidery Citywide on an as
needed contract basis, at a total annual amount not to exceed $85,000.00.
Creative Hands, the low bidder for Item No. 11 of the contract has failed to provide the shirts. RDH
Quest, Inc., who is a local vendor and is the next lowest responsible and responsive bidder to provide said
shirts is therefore, recommended for award.
The Purchasing Department has seen a larger need for this commodity than anticipated from various City
departments; and therefore an increase by $30,000.00 is necessary in order to meet the increasing demand
for this commodity.

amended to, 1) Rescind award to Creative Hands,
Inc., 2) Award bid to RDH QUEST, INC and 3)
Increase the contract by $30,000 for a total contract
amount not to exceed $115,000.00, for the provision
of T-shirts and Golf shirts with silk-screening and
embroidery Citywide; allocating funds therefore

No. 98-99-023 was mailed to 28 potential providers for said
shirts; and
WHEREAS, seven bids were received on February 22, 1999; and
WHEREAS, the City Manager and the Director of Purchasing
recommend that the bids `received from Alina Sportswear, Inc.,
Pentagon Health Care Services, Inc., Embroider It, Inc., Zipp
Sporting Goods, Creative Hands, and Miami To Go, Inc., be
accepted as the lowest responsive and responsible bids; and
