AN ORDINANCE OF THE MIAMI CITY COMMISSION
ESTABLISHING A NEW SPECIAL REVENUE FUND
ENTITLED: "STATE OF FLORIDA EMS (EMERGENCY
MEDICAL SERVICES) MATCHING GRANT PROGRAM
(FY '99-00)", AND APPROPRIATING FUNDS FOR THE
OPERATION OF SAME IN THE AMOUNT OF $32,500
CONSISTING OF A GRANT, IN THE AMOUNT OF
$24,375 FROM THE STATE OF FLORIDA DEPARTMENT
OF HEALTH AND MATCHING FUNDS, IN THE AMOUNT
OF $8,125, FROM CITY OF MIAMI OPERATING
BUDGET, ACCOUNT CODE NO. 001000.280601.6.840;
AUTHORIZING THE CITY MANAGER TO ACCEPT SAID
GRANT AND TO EXECUTE THE NECESSARY DOCUMENTS,
IN A FORM ACCEPTABLE TO THE CITY ATTORNEY,
FOR SAID PURPOSE; CONTAINING A' REPEALER
PROVISION AND A SEVERABILITY CLAUSE.
WHEREAS, the City of Miami has been awarded a grant, in the
amount of $24,375, from the State of Florida, Department of
Health, pursuant to §401.113(2)(b), Fla. Stat. (1999), to provide
grants for the improvement, expansion and. continuation of pre-
hospital emergency medical services ("EMS"); and
WHEREAS, matching funds from the City of Miami, in the
amount of $8,125 are required as a condition of said grant, and
is available from the City of Miami Operating Budget, Account
Code No. 001000.280601.6.840;
NOW, THEREFORE, BE IT ORDAINED BY THE COMMISSION OF THE CITY
OF MIAMI, FLORIDA:

Section 1. The recitals and findings contained in the
Preamble to this Ordinance are hereby adopted by reference and
incorporated as if fully set forth in this Section.
Section 2. The following Special Revenue Fund is hereby
established and resources are hereby appropriated as described
below:

accept said grant from the State of Florida, Department of
Health, and to execute the necessary document (s) in a form
acceptable to the City Attorney, for said purpose.
Section 4. All ordinances or parts of ordinances insofar
as they are inconsistent or in conflict with the provisions of
this Ordinance are hereby repealed.
�i The herein authorization is further subject to compliance with all
requirements that may be imposed by the City Attorney, including but not
limited to those prescribed by applicable City Charter and Code
provisions.

Section 5. If any section, part of section, paragraph,
clause, phrase or word of this Ordinance is declared invalid, the
remaining provisions of this Ordinance shall not be affected.
Section 6. This Ordinance shall become effective thirty
(30) days after final reading and adoption thereof2�

the Mayor vetoes this Ordinance, it shall become effective immediately

RECONEVIENDATION
It is respectfully recommended that the City Commission adopt the attached Ordinance establishing a new
Special Revenue fund entitled "State of Florida EMS Matching Grant Award (FY '99-00)" and
appropriating funds in the total amount of $32,500. The funds consist of a $24,375 grant apportioned by
the State of Florida Department of Health "EMS Matching Grant Program" and $8,125 matching funds
from the City of Miami Account Code 001000.280601.6.840.
BACKGROUND
The Florida Department of Health is authorized by Chapter 401, Part II, Florida Statutes, to provide -- -
grants to EMS Organizations to improve and expand pre -hospital emergency medical services ("EMS").
Therefore, the State of Florida Department of Health awarded a grant to the City of Miami entitled, "EMS
Matching Grant", in the amount of $24,375, to be apportioned to support the pre -hospital activities,
services, and other emergency medical services as specified in the Grant Application. Specifically, the
City will use these funds along with the matching funds allocated by the City of Miami for $8,125 to
equip our twenty-two (22) Advance Life Support (ALS) units with a combination of pulse oximeter/end
tidal CO2 units. This equipment is a valuable asset in the assessment and care of the approximately
18,000 respiratory or cardiac related alarms received by the City of Miami Department of Fire -Rescue
each year.

PROJECT DESCRIPTION AND JUSTIFICATION

6. State Plan: Brief synopsis and relationship to state plan goal, if applicable.
Our plan for this grant is to equip our 22 ALS units with combination pulse oximeter / end tidal
CO2 units. We plan to purchase 25 units, 22 for front line service, 1 for training, and 2 spares. .
This grant directly relates to the Emergency Medical Services State Plan Goal 14 of improving
morbidity and mortality outcomes in cardiac and stroke events. These instruments are a tool to
assist in the assessment and treatment of such patients, and will help guide the administration of
Oxygen to the most appropriate level, and assure that patients are correctly intubated.
7. Project Description/Justification: This is the NEED STATEMENT. Describe and justify the projebt.-
Include: (1) all available numerical data, time frames for the data, data source; (2) number :of
people directly impacted by the grant(s); (3) whether the project will serve single municipality,
county, multicounty, or regional area; and, (4) whether the project will coordinate with other EMS
organizations.
NEED STATEMENT:. (use only the space provided)
The City of Miami Department of Fire -Rescue operates 22 ALS Vehicles- in a 35 square mile
area Computer analysis of ourl998 reports showed there were over 17,000 respiratory or cardiac
related alarms and 1081 stroke calls (totaling over 31% of all EMS responses), where a pulse
oximeter would have been a valuable adjunct in the assessment and care of the patient. In
addition, the same analysis shows that 351 patients were intubated in 1998, and these would all
benefit from end tidal CO2 use, to confirm correct tube placement. The addition of this
equipment will serve any resident or visitor requiring EMS service, potentially any one of the
800,000 people that inhabit Miami daily. East Carolina University School of Medicine,
Department of Emergency Medicine's study, as reported in the Journal of Emergency Medicine
(1/14/96), concluded that 90% of the patients in their study had unrecognized, hypoxemia and
that 85% of those patients did not complain of respiratory distress. This study clearly points to a
need for this equipment, as the use of pulse oximeters on these patients would lead to quick
recognition and treatment of otherwise unrecognized respiratory problems. This program will
serve the residents and visitors of the City of Miami, and will not require coordination with other
EMS organizations, although our data on the use of the equipmen4-will be made available to any
interested EN1S provider.
8. Outcome measurable: Degree to which need will be met or changed. (use
provided) only the space
The outcome ,of this program will be easily. measurable. Analysis of run report data elements
such as respiratory treatments, and the percentage of patients treated and transported with
respiratory problems, will be monitored and compared to our historical data, and would be
expected to show a marked increase with the use of this equipment. Improved assessments with
corresponding.earlier intervention. will lead to improved mortality and morbidity rates.

Upon award of a grant the work activities would be first, the update of price quotes from vendors.
adjusting for any price fluctuations or product changes between the times of grant application .
and award This should take 3 weeks and is necessary to supply exact figures needed for the
preparation of City required legislation. The next step is to prepare the necessary legislation and
present the project to the City Commission, which because of agenda deadlines and other
bureaucratic delays in the system will take 9 weeks. The final action is the placement of purchase
orders for the products, and upon receipt of the equipment, the training necessary to place them
in service. This should take 3 months. The entire project will be finished, and the equipment
placed in service, within 6 months from the date of award.

ASSURANCES

PAYMENT FOR GRANT PROJECT: The grantee certifies, understands and accepts that due to state cash
flow and activity priorities, the grantee may not receive payment from the state for this activity until several
months after announcement of awards. The work activity time frames will be adjusted based on the date
payment is received, except the ending date of the grant will remain as specified in the Notice of Grant
Award letter.
STATEMENT OF CASH COMMITMENT: The. grantee certifies that the cash match will be expended between
the beginning and ending dates of the grant and will be used in strict accordance with the content of the
application and approved budget for this activity. No costs count towards satisfying a matching
requirement of a department grant if also used to satisfy a matching requirement of another state or
federal grant. Cash, salaries, fringe benefits, expenses, equipment, and other expenses as listed in this
application shall be committed and used for the department's final approved activity during the grant
period.
ACCEPTANCE OF TERMS AND CONDITIONS: The grantee accepts the grant terns and conditions in the
"Florida EMS Matching Grant Program Application Manual, and acknowledges this when funds are
drawn or otherwise obtained from the grant payment system.
DISCLAIMER: The grantee certifies that the facts and information contained in this application and any .:
attached documents are true and correct. A violation of this requirement may result in revocation of the
grant, return of all funds and interest to the department and any other remedy provided by law:
NOTIFICATION OF AWARDS: The grantee understands and accepts that the notice of award will be
advertised in the FAW, and that 21 days after this advertisement the grantee waives any right to
challenge or protest pursuant to chapter 120, F.S.
MAINTENANCE OF IMPROVEMENT AND EXPANSION: The grantee agrees that any improvement, expansion or
other effect brought about in whole or part by grant funds, will be maintained for five.years after the
activity ends, unless specified otherwise in the approved application or unless the department agrees in
writing to allow a change. Any unauthorized change within the five years will necessitate the return of
grant funds, plus interest.
