Section 1. The City Manager is authorized to accept a

It is respectfully recommended that the City Commission
adopt the attached Resolution accepting a grant in the total.
estimated amount of $200,000 from the U.S. Department of
Health and Human Services (under Title XX of the Social.
Services Act) through Miami -Dade County for the provision of
day care services to low-income families attending the
City's Daycare Program. The Resolution further authorizes
the City Manager to execute the necessary agreement for this
purpose.
BACKGROUND:
The Department of Parks and Recreation has prepared the
attached legislation_ concerning the Day Care Title XX Grant.
The City of Miami's Child Day Care Program is funded from
several sources, the City's General Fund, participant child
fees, the United States Department of Agriculture, and the
US Department of Health and Human Services Title XX of the
Social Services Act, The Title XX funding for 2000-2001,
amounts to approximately $200,000 and is channeled through
Miami -Dade County to the City on a reimbursement basis.
This is the 22nd year that the City will receive such funds,
and no cash match is required.
By means of this grant, the U.S. Department of Health and
Human Services purchases slots in City Day Care Centers in
order to provide child care services for children (ages 6
weeks - 5 years) of low income families, enabling their
parents to work and receive training outside the home.
Services will be provided for up to hundred and twelve (112)
children, forty (40) preschoolers, sixteen (47) toddlers and
four (4) infants.

Maximum daily rates paid to the City under this program are:
$19.00 for infant care, $18.00 for toddler care, and $17.00
for preschoolers. The total reimbursement amount is based
on the number and age of the children, the applicable rate
and the actual number of service days provided by the City.
Earlier this year, the City submitted an application to the
U.S. Department of Health and Human Services (through Miami -
Dade County) for the provision of daycare services to low
income families served by the City's Day Care Program. This
application was evaluated and approved by Miami Dade -County.
The City has received from Miami -Dade County the grant
agreement for execution. The contract period is from July
1, 2001 to June 30, 2002.

Attach Now New Price Schedule if applicable. (Price Schedule shalt be signed by Caterer before submitting to the
Institution.) Re -negotiation of price changes pertaining to the unit price shall not exceed the Consumer Price
Index as determined by the Florida Department of Health, Bureau of Child Nutrition Programs. No price
increase may be implemented under this provision without prior written approval of the Institution.
The Caterer certifies that he/she shah operate in accordance with all applicable State and federal regulations.
The Caterer certifies that all terms and conditions within the original Invitation to Bid contract shall be considered
a part of this renewal contract and incorporated therein.
This mutual agreement is solely between the Institution and Caterer. The Florida Department of Health, Bureau
of Child Nutrition Programs is in no way liable as an active participant in this renewal contract. The Department of
Health, Bureau of Child Nutrition Programs ads only in an advisory capacity.

• Renewal Instructions •
Prior to the expiration of the original contract, the Institution and caterer will need to complete the reverse side of
this form if both parties mutually agree to extend the terms and conditions of the original contract for an additional
year. Re -negotiation of price shall be the only. change (increases or decreases) allowed. Any price increases
shag not exceed the Consumer Price Index as determined by the Florida Department of Health. Bureau of Child
Nutrition Programs.
The Institution must indicate the Renewal Year and anticipated period to be covered by the Renewal Contract and
complete Columns 2 and 5 or a new Price Schedule (see Attachment 7. page 22 of the Invitation to Bid) before
submitting to Caterer. The Institution must not execute (sign) the Renewal contract prior to submitting to Caterer.
If needed, the Institution shalt submit to the caterer a new Delivery Schedule.
If the Caterer agrees to renew, the Caterer must complete columns 3 and 5 or a new Price Schedule (If a new
Price Schedule is completed, the Caterer must sign before submitting to the Institution.) The Caterer must
execute the Renewal Contract and submit to the Institution prior to the expiration of the original contract.
If the Institution agrees to renew, the Institution must execute the Renewal Contract. The original signed renewal
dontract shall be attached to the original Invitation to Bid contract. Each of the following parties shall retain one
copy of the renewal contract: the Institution; the Caterer; and the Bureau of Child Nutrition Programs.
Special Note: Original contracts may only be renewed for two (2) additional one (1) year periods.
