A RESOLUTION OF THE MIAMI CITY COMMISSION
APPROVING THE PROCUREMENT OF TWO COMPLETE
AERIAL BUCKET TRUCKS, WITH A 38 -42 -FOOT BOOM,
MODEL L42A, FROM AZTEC INDUSTRIES, INC. FOR
THE DEPARTMENT OF GENERAL SERVICES
ADMINISTRATION, UNDER EXISTING CITY OF KEY
WEST, FLORIDA, CES BID NO. 08-00, IN AN
AMOUNT NOT TO EXCEED $193,254; ALLOCATING
FUNDS FROM CAPITAL IMPROVEMENT PROJECT
NO. 311702.429401.6.840.
WHEREAS, the Department of General Services Administration
requires the procurement of two complete aerial bucket trucks
with a 38-42 foot boom, Model L42A, to replace aged and worn-out
trucks that are no longer cost effective to maintain and operate;
and
WHEREAS, the City Manager and the Director of the Department
of General Services Administration recommend the procurement of
two aerial bucket trucks from Al tec Industries, Inc., under
existing City of Key West, Florida, CES Bid No. 08-00, be
approved; and

WHEREAS, funds are available from Capital Improvement
Project No. 311702.429401.6.840;
NOW, THEREFORE, BE IT RESOLVED BY THE COMMISSION OF THE CITY
OF MIAMI., FLORIDA:
Section 1. The recitals and findings contained in the
Preamble to this Resolution are adopted by reference and
incorporated as if fully set forth in this Section.
Section 2. The procurement of two complete aerial bucket
trucks, with a 38 -42 -foot boom, Model L42A, from Altec
Industries, Inc. for the Department of General Services
Administration, under existing City of Key West, Florida, CES Bid
No. 08-00, in an amount not to exceed $193,254, is approved, with
funds allocated from Capital Improvement. Project
No. 311702.429401.6.840.
Section 3. This Resolution shall become effective
immediately upon its adoption and signature of the Mayor.11
1� If the Mayor does not sign this Resolution, it shall become effective at
the end of ten calendar days from the date it was passed and adopted.
If the Mayor vetoes this Reuolution, it shall become effective
immediately upon override of the veto by the City Commission.

It is respectfully recommended that the City Commission adopt the attached Resolution
authorizing the purchase of two (2) complete aerial bucket trucks (38-42 ft. boom) from ALTEC
Industries Inc. (non-minority/non-local vendor), located at 2570 Old Okeechobee Rd., West Palm
Beach, Florida 33409, through a bid awarded under an existing City of Key West , Florida, CES
Bid No. 08-00, in an amount not to exceed $193,254.00 Funds are available from Capital
Improvement Project No. 311702.429401.6.940.
BACKGROUND
The Department of General Services Administration seeks to purchase two (2) complete aerial
bucket trucks to be used by the Public Works Department. The Department of Public Works
currently has three aerial bucket trucks used for trimming trees in the right-of-way. These trucks
are old and are frequently in the shop for repairs. This requested purchase is necessary to replace
the aged aerial bucket trucks that are no longer cost effective to maintain and operate.

OVERCENTRAL AERIAL DEVICE
This specification is to set forth the specific requirements for a 42 It to bottom of
platform, hydraulic operated, articulating overcenter aerial device equipped with single,
side mounted platform and with a fibre line service body mounted on an appropriate
chassis/cab.
This aerial device shall be to the manufacturers standard. It shall be equipped with the
manufacturer's equipment and accessories which are included as standard in the
advertised and published literature for the unit. No such item of equipment or
accessories shall be removed or omitted for the reason that it was not specified in the
bid.
If it is necessary to bid alternate equipment or to take exceptions to the specifications as
set forth, this must be so stated in your bid. For each item, please place an X in the

SCHEDULE I — DIGGER DERRICK
SCOPE
This specification covers the requirements for City Electric System vehicular equipment
(digger derrick).
INSTRUCTIONS TO SUPPLIERS
Wherever used in the specifications, the letters "CES" shall mean City Electric System,
and the word "Supplier" shall mean the equipment supplier.
It is the intent of the specification to obtain a unit suitable to meet the needs of CES at
the lowest practical cost. The Supplier, therefore, is encouraged to submit an alternate
bid covering its standard unit and/or components which most closely approximate the
requirements of these specifications.
Before a purchase order is issued the Supplier shall furnish CES with a layout drawing
showing the basic components and body dimensions of the proposed vehicle.
The Supplier shall include all necessary transportation items (pick-up, delivery) in the bid
price. Upon satisfactory completion, the unit shall be delivered to the CES location
specified in the quotation request.
The completed unit shall be delivered with one (1) copy of the vehicle line ticket
(detailed vehicle parts listing) with two (2) complete sets of operation and maintenance
manuals for the vehicle and all major auxiliary equipment. The maintenance manuals
shall include a parts cross-reference listing which details manufacturer's part number
and, where applicable, equivalent original source name and part number is available.
These items are to be delivered to the CES representative at the time the vehicle is
accepted.
The apparent low bidder upon request shall demonstrate the proposed vehicle within
seven (7) working days after the bid opening date. The demonstration shall take place
at CES facilities or at a location not to exceed 125 miles from CES, Key West, Florida.
All items of material or details of construction which are not specified herein, or which
have not been commented on but which are normally furnished as part of a complete
unit shall be furnished by the Supplier. Should there be any doubt on the part of the
Supplier as to what shall be furnished under this specification, it shall be the Supplier's
responsibility to inform CES and obtain clarification in writing, prior to submitting a
quotation. Failure to do so shall in no way relieve the Supplier of the obligation to
furnish a unit as intended under the specification.

Standard of Design and Workmanship: All components shall be manufactured,
fabricated, assembled, and finished in a manner indicative of good workmanship and
high quality.

WARRANTY
The Supplier shall cover all warranty problems for parts and labor to cover all repairs
made necessary due to defective components and materials, poor workmanship, or
faulty design. The Supplier's qualified repairman shall perform any warranty work
incurred on CES jab location unless special shop facilities are a concern. Downtime due
to malfunctioning equipment is a cost item to CES and must be minimized. If downtime
is more than one (1) week during the duration of the warranty period, supplier shall
provide a vehicle equal to or greater for said period to CES, at no cost, until such
necessary repairs are made.
The bidder shall state the terms and conditions of the warranty in the bid proposal.
Starting date of the warranty shall be the date of acceptance of the completed
equipment by CES.
INSPECTION AND ACCEPTANCE
Final inspection, testing and acceptance of the completed unit will be made by CES
representatives at the location designated by CES.
LAWS AND REGULATIONS
All equipment, accessories, and material furnished shall be so designated and
constructed that when assembled or installed, the entire unit and all components will
comply with all applicable federal, state, and local laws, rules, and regulations.

CRANES, MATERIAL HANDLING TRUCKS, YARD CRANES, LINE TRUCKS
Particular attention is called to the Occupational Safety and Health Administration
(OSHA) Requirements for compliance with American National Standards Institute safety
standards for crawler, locomotive, and truck cranes (ANSI B30.5 -latest revision).
REFERENCES
The bidder shall submit a list of customers that are presently using the specified
equipment in the State of Florida.
MAINTENANCE
The bidder shall state the "Response Time" for service calls.
DETAILED QUOTATION
In addition to completing this specification, the bidder is to submit a detailed quotation of
the unit bid.
OPTIONAL STOCK UNIT
The bidder is encouraged to bid an optional stock unit that may be similar to these
specifications. Bidder must submit a detailed quotation of the unit bid.
FINAL SELECTION
CES reserves the right to select the lowest and best bid for the unit specified or the
optional stock unit. CES reserves the sole right to award the bucket truck and
digger/derrick to separate bidders.
BID FORM
The bidder is to specify the unit bid with appropriate model numbers. The bidder must
address each specification Item on this bid. For each item please mark "C" to denote
COMPLY or "NC" to denote NOT COMPLY. For each item marked "NC," please explain
the variance on a separate sheet with a reference to that particular item.
PUBLIC ENTITY CRIME CLAUSE
Any person submitting a bid or proposal in response to this invitation must execute the
