BE IT RESOLVED BY THE COMMISSION OF THE CITY OF MIAMI,
FLORIDA:
Section 1. The bids of Aero Products Corp. d/b/a
American Lafrance Medic Master, Emergency Medical Products, Inc.,
Pharmed Corp, Sunbelt Medical Supply, Inc. and Tri-Anim Health
Services, Inc. for the provision of intravenous (IV) fluids and
administration sets, for the Department of Fire -Rescue are
accepted, on an item -by -item, as -needed contract basis, for two
years with the option to renew for an additional one-year period,
at an estimated annual amount of $50,800, for a total contract

RECOMMENDATION
It is respectfully recommended that the City Commission adopt the attached Resolution accepting
the bids of Aero Products Corp d/b/a American LaFrance Medic Master, 3705 St. Johns
Parkway, Sanford, FL (Non-Minority/Non-Local vendor), Emergency Medical Products, Inc.,
1711 Paramount Court, Waukesha, WI (Non-Minority/Non-Local vendor), Pharmed Group,
3075 N.W. 107 Avenue, Miami, FL (Hispanic/Miami-Dade vendor), Sunbelt Medical Supply,
Inc., 20 Capital Drive, Hilton Head, SC (Non-Minority/Non-Local vendor), and Tri-Anim
Health Services, Inc., 13170 Telfair Avenue, Sylmar, CA (Non-Minority/Non-Local vendor) for
the provision of intravenous (IV) fluids and administration sets, on an as needed contract basis,
for two years with the option to extend for one additional one-year period, for the Department of
Fire -Rescue, in an estimated annual amount of $50,800, for a total contract award not to exceed
$152,400. Funds for this purchase will be allocated from the Department of Fire -Rescue
Account Code No. 001000.280601.6.714, subject to availability of funds.
BACKGROUND
The Department of Fire -Rescue has a need to acquire intravenous (IV) fluids and administration
sets on an as needed contract basis. This requested item is essential and is used by the Fire -
Rescue emergency personnel to administer medications to patient suffering from medical and
trauma related illness or injuries. Intravenous (IV) fluids are used to re -hydrate patients and the
administration sets are the tubing system that deliver the medications. For the safety and well
being of our citizens and visitors of the City of Miami, it is imperative that the Department of
Fire -Rescue is fully eauinned by assuring that a reliable source for intravenous (IV) fluids and
administration sets be secured.
The Department of Fire -Rescue has analyzed the bids received pursuant to Bid No. 02-03-019.
Bids submitted by Aero Products Corp d/b/a American LaFrance Medic Master, Emergency
Medical Products, Inc., Pharmed Group, Sunbelt Medical Supply, Inc., and Tri-Anim
Health Services, Inc. were deemed as the lowest responsible and responsive bids meeting
specifications and are therefore recommended for award.

This form is for use by any person serving at the county, city, or other local level of government on an appointed or elected board, council,
commission, authority, or committee. It applies equally to members of advisory and non -advisory bodies who are presented with a voting
conflict of interest under Section 112.3143, Florida Statutes.
Your responsibilities under the law when faced with voting on a measure in which you have a conflict of interest will vary greatly depending
on whether you hold an elective or appointive position. For this reason, please pay close attention to the instructions on this form before
completing the reverse side and filing the form.
INSTRUCTIONS FOR COMPLIANCE WITH SECTION 112.3143, FLORIDA STATUTES
A person holding elective or appointive county, municipal, or other local public office MUST ABSTAIN from voting on a measure which
inures to his or her special private gain or loss. Each elected or appointed local officer also is prohibited from knowingly voting on a mea-
sure which inures to the special gain or loss of a principal (other than a government agency) by whom he or she is retained (including the
parent organization or subsidiary of a corporate principal by which he or she is retained); to the special private gain or loss of a relative; or
to the special private gain or loss of a business associate. Commissioners of community redevelopment agencies under Sec. 163.356 or
163.357, F.S., and officers of independent special tax districts elected on a one -acre, one -vote basis are not prohibited from voting in that
capacity.
For purposes of this law, a "relative" includes only the officer's father, mother, son, daughter, husband, wife, brother, sister, father-in-law,
mother-in-law, son-in-law, and daughter-in-law. A "business associate" means any person or entity engaged in or carrying on a business
enterprise with the officer as a partner, joint venturer, coowner of property, or corporate shareholder (w er the; ar rfIhe rati

must disclose the nature of the conflict before making any attempt to influence the decision, whether orally or in writing and whether made
by you or at your direction.
