A RESOLUTION OF THE MIAMI CITY COMMISSION, AUTHORIZING THE
CITY MANAGER TO ACCEPT, BUDGET, AND APPROPRIATE A TOTAL
AMOUNT ESTIMATED TO BE ONE MILLION TWO HUNDRED FIFTY
THOUSAND NINE HUNDRED SIXTY-FOUR DOLLARS ($1,250,964.00)
("SERVICE FUNDS") FROM THE STATE OF FLORIDA'S AGENCY FOR
PERSONS WITH DISABILITIES ("APD") TO THE CITY OF MIAMI'S
DEPARTMENT OF PARKS AND RECREATION ("PARKS") FOR PARKS' LIFE
SKILLS DEVELOPMENT — LEVEL 3: ADULT DAY TRAINING ("ADT")
PROGRAM AND TRANSPORTATION SERVICES FOR PERSONS WITH
DISABILITIES (COLLECTIVELY, "SERVICES AND PROGRAMS");
ESTABLISHING A NEW SPECIAL REVENUE PROJECT ACCOUNT TITLED
"PROGRAMS FOR THE DEVELOPMENTALLY DISABLED 2021-2026" TO
FUND SERVICES AND PROGRAMS FOR THE DEVELOPMENTALLY
DISABLED ADMINISTERED BY PARKS; AUTHORIZING THE CITY MANAGER
TO CREATE A NEW PROJECT NUMBER FOR THE SERVICES AND
PROGRAMS, TO CREATE THE NECESSARY BUDGETING AND
ACCOUNTING STRUCTURE TO ACCOUNT FOR THE SERVICE FUNDS IN
ACCORDANCE WITH THE GENERALLY ACCEPTED ACCOUNTING
PRINCIPLES, AND TO COMPLY WITH APD REQUIREMENTS; FURTHER
AUTHORIZING THE CITY MANAGER TO NEGOTIATE AND EXECUTE A
MEDICAID WAIVER PROVIDER RENEWAL AGREEMENT ("AGREEMENT")
WITH THE APD, IN A FORM ACCEPTABLE TO THE CITY ATTORNEY, AND
TO NEGOTIATE AND EXECUTE ANY AND ALL OTHER NECESSARY
DOCUMENTS, AMENDMENTS, EXTENSIONS, RENEWALS, DELEGATIONS
OF AUTHORITY, AND MODIFICATIONS TO THE AGREEMENT, ALL IN
FORMS ACCEPTABLE TO THE CITY ATTORNEY, TO IMPLEMENT THE
ACCEPTANCE OF AND COMPLIANCE WITH THE SERVICE FUNDS FOR
SAID PURPOSES FOR THE PERIOD COMMENCING JULY 1, 2021 AND
ENDING JUNE 30, 2026 FOR THE SERVICES AND PROGRAMS.

WHEREAS, the City of Miami ("City"), through the Department of Parks and Recreation
("Parks"), obtains funds to support Parks' Life Skills Development — Level 3: Adult Day Training
("ADT") Program and Transportation Services (collectively, "Services and Programs") through
the State of Florida's Agency for Persons with Disabilities ("APD"); and
WHEREAS, APD is providing funds to the City, on a reimbursement basis, in a total
amount estimated to be One Million Two Hundred Fifty Thousand Nine Hundred Sixty -Four
Dollars ($1,250,964.00) ("Service Funds") for a five (5) year period commencing July 1, 2021

and ending June 30, 2026 for the Services and Programs provided within the City by Parks to
developmentally disabled individuals; and
WHEREAS, it is in the best interest of the City for the City Commission (a) to authorize
the City Manager to accept, budget, and appropriate the Service Funds and to establish a new
special revenue project account titled "Programs for the Developmentally Disabled 2021-2026";
(b) to fund the Services and Programs for the developmentally disabled administered by Parks;
(c) to authorize the City Manager to create a new project number for the Services and
Programs; (d) to create the necessary budgeting and accounting structure to account for the
Service Funds in accordance with the generally accepted accounting principles; and (e) to
comply with APD's requirements; and
WHEREAS, the City Manager and Parks are requesting for the City Commission to
authorize the City Manager to negotiate and execute a new Medicaid Waiver Service Renewal
Agreement ("Agreement') with APD, in a form acceptable to the City Attorney, and to negotiate
and execute any and all other necessary documents, amendments, extension, renewals,
delegations of authority, and modifications to the Agreement, all in forms acceptable to the City
Attorney, to implement the acceptance of and compliance with the Service Funds for said
purposes for the period of July 1, 2021 through June 30, 2026 for the Services and Programs;
NOW THEREFORE BE IT RESOLVED BY THE COMMISSION OF THE CITY OF
MIAMI, FLORIDA:
Section 1. Recitals and findings contained in the Preamble of this Resolution are
adopted by reference incorporated as if fully set forth in this section.
Section 2. The City Manager is authorized' to accept, budget, and appropriate the
Service Funds for the Services and Programs for persons with disabilities.
Section 3. The City Manager is authorized to establish the following new Special
Revenue Project Account titled "Programs for the Developmentally Disabled 2021-2026" to fund
the Services and Programs for the developmentally disabled administered by Parks:

Section 4. The City Manager is authorized' to create a new project number for the
Services and Programs, to create the necessary budgeting and accounting structure to account
for the Service Funds in accordance with the Generally Accepted Accounting Principles, and to
comply with the APD's guidelines.
Section 5. The City Manager is authorized' to negotiate and execute an Agreement with
the APD, in a form acceptable to the City Attorney, and to negotiate and execute any and all
other necessary documents, amendments, extensions, renewals, delegations of authority, and
modifications to the Agreement, all in forms acceptable to the City Attorney, to implement the

acceptance of and compliance with the Service Funds for said purposes for the period
commencing July 1, 2021 and ending June 30, 2026 for the Services and Programs.
Section 6. This Resolution shall become effective immediately upon its adoption and
signature of the Mayor.2

from the date it was passed and adopted. If the Mayor vetoes this Resolution, it shall become effective
immediately upon override of the veto by the City Commission.
